Community Occupational Therapist supporting adults from hospital to home through functional assessments, rehabilitation planning, and client-centered care. This position offers full time or part-time work.

At St. Joseph's Home Care, you will support adults with complex medical conditions in home and community settings. You will assess cognitive, physical, perceptual, and emotional function, then develop individualized treatment plans. Collaboration with families and interdisciplinary healthcare teams supports discharge planning and care coordination.

Key Responsibilities:

- Conduct holistic assessments of adult client functioning
- Develop and implement individualized rehabilitation treatment plans
- Educate clients and caregivers about exercises and equipment




- Coordinate discharge planning with interdisciplinary healthcare teams
- Document care activities and maintain accurate records

Requirements:

- Bachelor's or Master's Degree in Occupational Therapy
- Registration with the College of Occupational Therapists of Ontario
- Current BCLS/HCP certification
- One to three years of clinical experience
- Experience with complex medical conditions and mobility support
