Trillium Health Partners seeks a Registered Nurse for McCall Centre Rehab’s 120-bed chronic care facility. You will support transitional patients and families through assessment, care planning, goal-oriented interventions, and discharge planning. The position serves Primary, Rehab, CCC & Seniors programming.

Key Responsibilities:

- Manage nursing care decisions and actions for patients
- Complete nursing assessments and contribute to care plans
- Provide treatments, education, and family support
- Contribute to realistic discharge planning
- Coach teams and support unit clinical leadership

Requirements:

- Graduate from a recognized School of Nursing
- Hold a required baccalaureate degree
- Maintain Ontario nursing certification or eligibility
- Use sound critical thinking skills
- Availability for days, nights, and 12-hour shifts
