Seabird Island seeks a relationship-focused Case Manager for a full-time term contract. Provide client-centered support to residents navigating substance use, addictions, and life transitions. Collaborate with community partners and health professionals while applying culturally safe, Indigenous-informed practices.

Key Responsibilities:

- Develop individualized wellness and recovery plans for residents
- Provide case management, advocacy, referrals, and ongoing support
- Facilitate addictions recovery and trauma-informed care groups
- Coordinate housing, healthcare, education, employment, and financial resources




- Maintain confidential records, case notes, and resident documentation

Requirements:

- Diploma in social services, addictions, mental health, or related field
- Minimum three years of case management or related experience
- Understanding of Indigenous cultures and intergenerational trauma impacts
- Solid communication, relationship-building, and conflict resolution skills
- Class 5 licence, reliable transportation, and required certifications
