As a Home Support Supervisor, you will help deliver integrated, personalized support through Spectrum's Integrated Care Program. The position involves community-based leadership, patient care planning, staff management, and outcome monitoring. You will collaborate with multidisciplinary teams while supporting care delivery in Ontario.

Key Responsibilities:

- Travel to patients' homes throughout the community
- Assess patient needs and formulate individualized care plans
- Manage staff, appraisals, and progressive disciplinary action
- Analyze patient and employee feedback with key indicators




- Address and document patient concerns and complaints promptly

Requirements:

- Current College of Nurses of Ontario registration in good standing
- At least three years' progressive leadership experience
- Valid driver's license and reliable vehicle access
- Solid interpersonal and difficult-situation management skills
- Excellent English communication and community nursing knowledge
