The Community Social Worker delivers direct services through intake, assessments, case management, referrals, and follow-up. Working with the Stoney Trail Wellness Centre interdisciplinary team, this full-time position coordinates care with families, agencies, government departments, and service providers. The work strengthens access, continuity, self-determination, and independence.

Key Responsibilities:

- Conduct client intake and ongoing needs assessments
- Develop individualized care and service plans
- Coordinate referrals and monitor progress toward goals
- Assist with applications, documentation, and appointments
- Maintain confidential client records and skilled boundaries

Requirements:

- Bachelor of Social Work preferred; diploma may be considered
- Current Alberta College of Social Workers registration
- Three to five years relevant professional experience
- First Aid and CPR certification
- Valid Class 5 licence and required checks
