Reporting to the Manager, this Social Worker develops care plans using social work theory and methods. The work addresses barriers affecting recovery, adjustment, hospital discharge, independence, and coping capacities. Support also extends to end-of-life situations and interdisciplinary care.

Key Responsibilities:

- Conduct bio-psycho-social assessments for patients
- Develop care plans with the interdisciplinary team
- Address barriers to recovery, adjustment, and discharge
- Support patients in end-of-life situations
- Apply social work theory and methods

Requirements:

- Master of Social Work degree
- Current registration in good standing with the College of Social Workers
- Two to five years of recent medical clinical experience
- Evidence-based counselling and supportive therapy competence
- Availability for all shifts, including weekends
