Support resident nutrition and hydration care as a part-time Registered Dietitian at Extendicare in Lakefield, Ontario, using assessment and care planning skills.

The Registered Dietitian provides nutrition and hydration care for residents at the Lakefield location. This position requires assessing nutrition status, determining risk, developing care plans, and documenting interventions. You will also counsel residents, families, caregivers, and substitute decision makers while collaborating with interdisciplinary teams.

Key Responsibilities:

- Assess resident nutrition status during admissions, annually, and after significant changes
- Determine nutrition and hydration risks and recommend interventions
- Develop, evaluate, and modify resident nutrition care plans




- Document nutrition care and monitor residents requiring ongoing follow-up
- Review menus and oversee restorative feeding and enteral nutrition programs

Requirements:

- University degree in Food and Nutrition from an approved university
- Current registration with the applicable provincial College of Dietitians
- One to two years of gerontology or long-term care experience is an asset
- Membership in Dietitians of Canada and the Gerontology Network
- Solid communication, critical thinking, coordination, and computer skills
