Support residents through individualized care planning and clinical assessments as an RPN with Cogir Senior Living’s resident-centred care team.

This Registered Practical Nurse role focuses on personalized services for seniors in independent living, assisted living, and memory care. You will coordinate planned, goal-directed nursing care while communicating effectively with residents, families, and multidisciplinary teams. Accurate documentation and evaluation remain central.

Key Responsibilities:

- Deliver patient-oriented nursing care to residents
- Assess residents and maintain current clinical documentation
- Evaluate care plan effectiveness and make required changes
- Communicate resident changes to families and teams
- Direct and mentor unregulated care providers

Requirements:

- Registration as a Registered Practical Nurse in the applicable province
- Experience with seniors preferred
- Experience with unregulated care providers preferred
- Robust teamwork, customer service, and relationship-building abilities
- Current Vulnerable Sector Check or willingness to obtain one
