Apply Housing First and harm reduction practices as a Mobile Outreach Worker supporting Pacifica Housing tenants across sites and community settings. The schedule includes Thursday through Saturday and every other Wednesday.

This position provides flexible, ongoing support for tenants navigating mental health, substance use, poverty, and social isolation. The worker builds professional relationships while encouraging resilience, autonomy, housing stability, and community connection. Responsibilities span advocacy, crisis intervention, wellness support, tenancy responsibilities, and individualized goal planning.

Key Responsibilities:

- Monitor tenant mental health and substance use needs
- Provide overdose response, naloxone administration, and emergency support
- Support transitions into or out of Pacifica Housing properties




- Encourage participation in wellness, recreational, and life skills programs
- Provide backup support and participate in interdisciplinary meetings

Requirements:

- Experience with homelessness, addictions, mental health, or housing barriers
- Conflict resolution or crisis prevention training and experience
- Knowledge of psychosocial rehabilitation and Housing First theory
- Ability to work independently within an integrated community team
- Proficiency with Word, Outlook, Excel, and mobile communication tools
