Bring social work expertise to UHN’s Toronto General Hospital, supporting inpatient patients through assessment, intervention, and community coordination. This casual role works days.

This Social Worker supports weekday and weekend inpatient acute care programs at Toronto General Hospital. Under the Manager’s direction, you will collaborate with an interdisciplinary team while evaluating service needs and psychosocial functioning. You will also develop recommendations, treatment plans, and discharge plans for patients.

Key Responsibilities:

- Evaluate patient needs and psychosocial functioning
- Formulate recommendations, treatment plans, and discharge plans
- Coordinate interventions with patients and family members




- Exchange patient information with community agencies
- Support effective operation of the Social Work group

Requirements:

- Master’s degree in Social Work required
- Registration with Ontario’s social work college required
- Hospital acute care experience required
- Robust interpersonal, organizational, and time management skills
- Social welfare policies, legislation, and services knowledge
