West Bedford’s recent Transitional Health team needs a part-time Registered Nurse skilled in patient assessment and care planning. Help patients return home safely.

At Shannex, the RN provides specialized, person-centred support for patients needing additional care after hospital treatment. You will use evidence-based clinical expertise, technology, and communication to align care with patient and family goals. Reporting to the Manager of Transitional Services, you will collaborate across disciplines while supporting dignity, safety, and clinical excellence.

Key Responsibilities:

- Develop, implement, and evaluate individualized care plans
- Assess patients and maintain quality of life
- Administer and record medications under facility policy




- Ensure care tasks and incidents are documented accurately
- Support quality improvement, risk management, and assurance activities

Requirements:

- Bachelor of Science in Nursing
- Current provincial Registered Nurse registration
- Valid CPR and First Aid Certification
- Supervisory skills and employee performance evaluation ability
- Experience with complex admissions and discharges is an asset

Use assessment, care planning, documentation, and interprofessional collaboration to support successful home and community transitions.
