This Care Supervisor position coordinates referrals, reassessments, care plans, and service quality. You ensure Care Providers have the knowledge and skills required for individual patient needs. You also investigate complaints, respond to incidents, and maintain policy-aligned documentation and reporting.

Key Responsibilities:

- Reassess patients during interim visits
- Ensure care plans meet individual patient needs
- Confirm Care Providers can fulfill assigned plans
- Investigate, resolve, and report customer complaints
- Respond comprehensively to incident reports

Requirements:

- Current CNO registration as a Registered Nurse or Registered Practical Nurse
- Two years supervisory or community service delivery experience
- Robust rapport-building abilities with clients and Care Staff
- Valid Ontario driver’s license with reliable vehicle access
- Problem-solving aligned with policies and best practices
