Deliver acute-level home care in Newmarket using advanced assessment, complex treatments, and remote monitoring. This part-time position supports Southlake Health patients.

CBI Home Health seeks a Registered Nurse for its Integrated Care Program with Southlake Health. You will provide hospital-level nursing care at home for patients with acute and complex needs. The work includes clinical assessment, IV therapy, medication administration, wound care, and point-of-care diagnostics.

Key Responsibilities:

- Deliver advanced assessments and complex treatments in patients' homes
- Monitor acute conditions using remote health technologies
- Recognize patient changes and coordinate timely care
- Collaborate with hospital and community partners




- Support secure recovery and avoidable admission prevention

Requirements:

- Registered Nurse in good standing with the College of Nurses of Ontario
- Experience providing acute, complex, or hospital-level nursing care
- Experience with IV therapy and vascular access devices
- Current valid BCLS training and required screening documentation
- Valid driver's license, reliable vehicle, and vehicle insurance
