Cota Health is seeking a Case Manager, Addictions Specialist to provide Recovery Oriented services to clients in encampments in East Toronto. The role supports housing, health access, and education within an Assertive Housing First framework.

You will conduct assessments, develop safety and recovery plans, and coordinate community resources, with a focus on harm reduction and client-centred care in a agile outreach setting.
