You serve as the primary clinical resource for designated Community Health Workers, helping clients remain protected, independent, and engaged at home. The position combines in-home assessments, client-specific delegation, care-plan oversight, and family communication. It also requires organized documentation, electronic records, and collaboration with Scheduling, Operations, and healthcare providers.

Key Responsibilities:

- Assess changing client conditions and coordinate care-plan adjustments
- Provide real-time clinical guidance and problem-solving support
- Evaluate CHW competence in transfers, medications, and infection control
- Investigate incidents, adverse events, falls,



and medication errors
- Represent Hero Home Care with healthcare and community partners

Requirements:

- Licensed Practical Nurse registration with the BC College of Nurses and Midwives
- Active Basic Life Support or healthcare-provider CPR certification
- Strong assessment skills in chronic disease, wounds, falls, and mobility
- Proficiency with lifts, oxygen equipment, catheters, and tube feeding
- Confidence using electronic medical records and digital calendars
