Perennial is seeking a part-time Nurse Practitioner to join our Toronto clinical team, starting November 2026. The role offers 1–4 days per week with fixed clinic days to ensure continuity of care for residents in retirement residences.

You will provide comprehensive primary care in on-site and cooperative settings, conducting check-ups, managing chronic/acute conditions, ordering tests, prescribing medications, and developing care plans.
