ParaMed home health care needs a Registered Nurse skilled in patient assessments and care planning. Deliver scheduled visits while supporting comfort in patients’ homes.

This community nursing position serves patients of all ages and differing complexity of care needs. You will assess individualized needs, execute and monitor care plans, and engage patients through rapport and trust. You will also update skilled records, travel to residences within a designated area, and provide after-hours on-call coverage for patient safety.

Key Responsibilities:

- Deliver scheduled nursing visits in patient homes and settings
- Use assessment techniques to identify individualized care needs
- Continuously monitor care plans and patient responses




- Engage patients through collaboration, rapport, and trust
- Update records documenting encounters and care plans

Requirements:

- Current CNO Registered Nurse registration in good standing
- Valid driver’s license with access to a vehicle
- Reliability and flexibility for community nursing schedules
- Willingness to travel between patient residences
- Required on-call availability, including after hours
