Support rehabilitation through occupational and physical therapy interventions with Vancouver Coastal Health’s Groundspring Home Health team in Vancouver. Work from Lily Lee Community Health Centre.

As a Rehabilitation Assistant, you work within client and family centred care under licensed occupational therapist or physiotherapist direction. You support clients with acute, chronic, palliative, or rehabilitative needs while promoting independence. The position includes care-plan support, community transitions, family direction, observations, equipment safety, and inventory management.

Key Responsibilities:

- Provide assigned occupational and physical therapy therapeutic interventions
- Assist with modifications to occupational and physical therapy care plans
- Facilitate transitions to community programs




- Provide functional direction and self-care education to families
- Document observations and manage equipment safety and inventory

Requirements:

- Recognized diploma rehabilitation assistant program with occupational and physical therapy components
- One year of recent related experience with target populations
- Valid British Columbia driver’s licence and personal vehicle
- Effective verbal and written communication skills
- Computer literacy, organization, collaboration, and problem-solving abilities
