The Housing First Case Management Worker role supports individuals and families facing housing barriers to secure and maintain protected housing in the community. It uses a case management approach to identify needs and connect clients with services, housing opportunities, government and local resources.

Responsibilities include developing case plans to address housing barriers, assisting with access to resources and supports, and building capacity for independent, stable living in the community.
