Local 1 CanadaBasic Function and Scope: The Resident and Family Liaison provides social and emotional support to residents and their families to promote adjustment, communication, and well-being within the long-term care home. Working collaboratively with the interdisciplinary team, the Liaison assists residents and families in navigating transitions, accessing services, and fostering a positive and inclusive home environment in alignment with the Fixing Long-Term Care Act, 2021.Hours of Work: Monday to Friday, daysPay Rate: $28.06-$32.30Qualifications & RequirementsEducation: Diploma or equivalent in Social Services Work or related human services field from a recognized institution.Registration: Eligibility or registration (preferred) with the Ontario College of Social Workers and Social Services Workers (OCSWSSW).Experience: Experience with older adults, LTC, community, or health/social sector preferred.



Basic knowledge of the Fixing Long-Term Care Act, 2021, the Residents’ Bill of Rights, and LTC admission/discharge processes.Required Skills: Robust Communication Skills, Interpersonal, and organizational skills. Ability to work collaboratively in a multidisciplinary team.
