NEOFACS seeks a full time, twelve-month contract case manager within its Child and Youth Mental Health team. You will collaborate with service providers and manage intensive coordination for identified children and youth with multiple or complex special needs. The position serves voluntary services funded by Ontario ministries across rural, remote, and urban communities.

Key Responsibilities:

- Develop integrated service plans for children, youth, and families
- Coordinate and manage integrated plan implementation
- Provide comprehensive intensive service coordination functions




- Collaborate with multidisciplinary service providers
- Address identified child, youth, and family needs

Requirements:

- Graduate or baccalaureate degree in Human Services
- Excellent case management knowledge and skills
- English and French bilingualism required
- G2 or G Class driver’s license and vehicle access
- Flexible hours and travel as required
