Deliver proactive patient-centred care as a Clinical Pharmacist with Mint Health + Drugs Meridian in Stony Plain, Alberta. Use prescribing and injections daily.

This full-time, permanent Pharmacist role supports an established Pharmacist First workflow. Provide medication reviews, chronic disease management, dispensing, verification, and follow-up care. You will also support travel health services while collaborating with pharmacy technicians, assistants, physicians, primary care networks, and community partners.

Key Responsibilities:

- Provide medication reviews, prescribing, injections, and chronic disease management
- Assess patients proactively and develop follow-up care plans
- Support the appointment-based travel clinic and clinical services




- Dispense and verify prescriptions accurately and safely
- Collaborate with pharmacy technicians, assistants, and community partners

Requirements:

- Clinical pharmacist registration with the Alberta College of Pharmacists
- Authorization to administer drugs by injection
- Additional Prescribing Authorization
- At least one year of community pharmacy experience
- Robust communication, organization, and patient-centred care commitment
