Vancouver Coastal Health is seeking an Occupational Therapist/Physiotherapist Case Manager to join the Home Health team, based at the Three Bridges Community Health Centre. You will deliver client- and family-centred case management in home and community settings, coordinating care plans and collaborating with an interdisciplinary team.
The role requires experience with geriatric or chronic illness rehabilitation, solid assessment skills and knowledge of community resources.
