In this role, the Social Worker will facilitate psychosocial assessments of the emotional, physical, cognitive, and psychosocial needs of patients with complex/chronic medical needs and their families. As an integrated member of the interprofessional team, the Social Worker will provide discharge planning, referrals and resource navigation, advanced care planning and counseling to patients and their families, with a focus on ensuring successful transitions in care. The Social Worker will facilitate referrals to Ontario Health atHome Services,



along with community resource navigation as required. The Social Worker in this position demonstrates a high degree of practice excellence, and functions as a clinical resource for the interprofessional team as well as the patient and family. This position reports directly to the Manager and Interprofessional Practice Chief for Social Work.Key ResponsibilitiesProvides psychosocial assessments, crisis intervention, discharge planning and counseling to patients and their familiesProvides support/consultation to other disciplines on the team re: psychosocial issues particularly related to complex patient/family situations and transitions to the communityActs as the team expert on discharge planning and patient flowActs as the team liaison with Ontario Health at HomeActs as a linkage and communication conduit between the family and the care teamEstablishes community linkages and outreach with relevant agencies and other Baycrest programs and servicesParticipates in teaching and other student supervision activities for health professional students assigned to this unitKey QualificationsMasters of Social Work degreeCurrent and valid registration with the Ontario College of Social Workers and Social Service Workers (OCSWSSW)Experience in discharge planning and system navigation preferredExperience working in a healthcare setting preferredExperience working with older adults with complex medical, cognitive, physical and emotional needs preferredExcellent interpersonal, communication and problem-solving skillsBasic computer skills – Meditech, Microsoft Word and Excel, RM&R; preferred#J-18808-Ljbffr
