This Intensive Case Manager provides direct service within Positive Shepherd Non-Profit Homes. The work supports clients facing mental illness, substance use, and barriers affecting health, wellbeing, vocation, or education. You will build trust, assess needs, and collaborate on client-driven recovery goals.

Key Responsibilities:

• Maintain an Intensive Case Management caseload within provincial guidelines
• Complete intake interviews, assessments, reports, files, and progress notes
• Provide resource information, referrals, advocacy, and community facilitation
• Support life skills, competencies, self-sufficiency,

• Apply psychosocial rehabilitation and strengths-based empowerment approaches

Requirements:

• University degree in Social Work, Nursing, Occupational Therapy, or related field
• Addictions Certificate and two years of related work experience
• Experience supporting problematic substance use and mental health challenges
• Crisis intervention, risk assessment, de-escalation, and problem-solving skills
• CPR certification or willingness to become certified
