Positions require 2+ years in behavioral health or ICU/ED, with preference for prior managed care case management experience; bilingual is a plus.
Care managers will conduct assessments, develop care plans, and coordinate services with the care team.
Telephonic work with occasional in-person visits, standard hours 8 AM–5 PM, in local time zone; travel up to 25–40% may be required.
