Williamsville Suburban seeks an experienced full‑time MDS Coordinator to oversee the entire patient assessment process and ensure timely, accurate assessments and care plans.

RAC‑CT preferred; MDS certification and PDPM/OBRA knowledge are required.

Detail oriented, able to work independently while maintaining high accuracy; training available for qualified candidates.

The position offers four days on site and one day remote, market-market-competitive salary with full perks and a $5,000 signing bonus after
