The position includes comprehensive assessments, holistic care planning, nursing standards, infection prevention, and interdisciplinary collaboration. Key Responsibilities: Provide high-quality, in-home client-centered care
Complete comprehensive client assessments
Develop collaborative, holistic care plans
Follow CNO standards and AHS policies




Participate in on-call nursing rotation
Requirements: Registered Nurse or Registered Practical Nurse in good standing
Valid driver’s license and required vehicle
Two-million-dollar liability insurance coverage
Valid CPR, recent TB test, and police check
Excellent assessment, documentation, verbal, and communication skills
