LOFT Community Services seeks a Psychogeriatric Case Manager for its 24/7 Assisted Living-Dunn program. Provide direct support to seniors with serious mental illness, addictions, cognitive impairment, and complex physical health challenges. Build community linkages supporting independence, dignity, and self-respect.
Key Responsibilities: Develop individualized care plans and connect clients with appropriate resources
Conduct RAI and OCAN assessments with accurate documentation
Provide counselling, crisis intervention, and service navigation
Coordinate care with families, caregivers, teams, and providers




Advocate for clients while maintaining confidentiality and professional standards
Requirements: Post-secondary education in Social Services, Human Services, or equivalent
At least three years supporting vulnerable seniors and older adults
Knowledge of community resources, counselling, assessment, and engagement
Understanding of harm reduction and psychosocial rehabilitation principles
Vulnerable sector check, vehicle access, and occasional flexible schedule
Bring assessment, counselling, and crisis intervention expertise to older-adult services with LOFT Community Services in Toronto.
