You will receive referrals, assess eligibility, and coordinate services. Key Responsibilities: Receive referrals and assess Home Care support eligibility
Develop and evaluate client plans of care
Coordinate discharge support from acute care settings
Plan alternate care or personal care home arrangements
Maintain liaison with health services and families
Requirements: B.S.W. or BN with current appropriate registration
Two years' Home Care Case Coordinator experience
Acute care discharge planning experience required
Knowledge of community resources and Home Care services
Valid Manitoba Class 5 licence and reliable vehicle
