Key Responsibilities: Assess care needs and determine eligibility with patients and families
Develop individual care and service plans
Link patients with appropriate service providers
Coordinate and monitor care plan delivery
Balance patient preferences with available resources
Requirements: Current Ontario regulatory membership as nurse or allied qualified
Two or more years’ recent community health experience
Knowledge of health care systems and community resources
Strong assessment, communication, and decision-making skills
Valid driver’s licence and reliable vehicle access
