Community Registered Nurses deliver skilled home care through patient assessments and individualized care plans during scheduled visits. This full-time position serves patients across various settings.
At ParaMed, you will care for patients of all ages with differing complexity of needs. You will help patients remain comfortable at home while building long-term therapeutic relationships. The position requires community visits, record maintenance, and after-hours on-call coverage.
Key Responsibilities: Assess individualized patient needs using appropriate clinical tools
Execute and continuously monitor patient care plans
Build rapport and trust through collaborative patient engagement
Maintain and update professional patient records




Provide required after-hours on-call coverage for patient safety
Requirements: Current positive-standing Registered Nurse registration with the CNO
Valid driver’s license and access to a vehicle
Community care experience considered an asset
Reliability and flexibility for scheduled community visits
Willingness to travel between patient residences
Bring registered nursing expertise, assessment skills, and care planning experience to ParaMed’s community home care services.
