CMHA HKPR seeks a relief Mental Health Housing Worker for a 24/7 supportive housing environment. This position supports people affected by serious mental illness, acquired brain injury, dual diagnosis, substance use, and housing instability. You will assess needs, evaluate risks, develop safety plans, and coordinate treatment goals with clients and identified supports. The role also includes system navigation across health, housing, financial, legal, education, and employment services. Work occurs in offices, communities, clients’ homes, and residential settings, with rotating days, evenings, overnights, weekends, and holidays. Successful candidates bring case management experience, sound judgment, observation skills, and solid communication. A social or health science degree is preferred,



while a relevant diploma with significant demonstrated experience may be considered.
Key Responsibilities: Assess client needs and develop strategies to mitigate risks
Create safety plans and communicate them with supports
Coordinate treatment goals, referrals, and service planning
Advocate and negotiate on behalf of clients
Monitor treatment progress and maintain client records
Requirements: Minimum three years’ mental health case management experience
Understanding of crisis theory, recovery, and psychiatric medication
Ability to work independently and collaboratively with teams
Proven assessment, decision-making, and conflict-resolution abilities
Completion of required training and satisfactory screening
Bring case management, risk assessment, and collaborative service-planning expertise to clients supported by HART Hub.
