Job Title: Community Support Worker (Complex Care)

About theRole
We areseeking compassionate and dedicated Community Support Workers to join our teamin a medically supported residential home. In this role, you will supportindividuals with developmental disabilities and complex medical needs whorequire assistance with all aspects of daily living, mobility, and personalcare.

Thisposition offers a unique opportunity to work closely with an interdisciplinaryteam, including on-site nurses available 24 hours a day. It is ideal fornursing students, Health Care Assistants, recent healthcare graduates, orindividuals looking to build experience in complex care, community health, anddisability services.

Dutiesand Responsibilities:

Provideperson-centered care that promotes dignity, independence, comfort, and qualityof life.

Assistindividuals with all aspects of personal care, including bathing, dressing,grooming, toileting, continence care, peri-care, repositioning, and skin care.

Supportindividuals with mobility needs, including wheelchair assistance, transfers,and the safe use of mechanical and overhead lifts.

Assistwith mealtime support and specialized feeding plans. Experience withG-tube/J-tube feeding is considered an asset.

Workcollaboratively with on-site nursing staff to follow individualized care plansand support medical and health-related needs.

Monitorand report changes in individuals' physical, emotional, or behavioralwell-being to the nursing team and leadership staff.





Completedaily documentation, medication records, and incident reporting in accordancewith organizational policies and procedures.

Supportindividuals in recreational, social, and community activities wheneverappropriate.

Assistwith household duties including meal preparation, laundry, light housekeeping,and maintaining a clean, safe living environment.

Qualifications

CommunitySupport Worker (CSW) Diploma, Health Care Assistant Certificate, nursingeducation, or a combination of relevant education and experience.

ValidClass 5 BC Driver's Licence.

Class4 Driver's Licence is considered a strong asset.

CurrentFirst Aid with CPR-C.

ValidTB Test.

MANDTCertification (or willingness to obtain).

SatisfactoryCriminal Record Check.

Skillsand Abilities

Previousexperience supporting individuals with disabilities, complex medical needs,



orin healthcare settings is considered an asset.

Experiencewith G-tube/J-tube feeding, tracheostomy care, or other medical supports is anasset.

Experienceusing overhead lifts, mechanical lifts, transfer equipment, and mobilitydevices is an asset.

Strongunderstanding of person-centered care and trauma-informed practice.

Abilityto provide full personal care support with professionalism, respect, andcompassion.

Abilityto work independently while collaborating effectively with nurses andinterdisciplinary team members.

Strongcommunication, observation, and documentation skills.
