Mental health social work leads this full time Social Worker II position at Claresholm Centre for Mental Health and Addictions. Deliver patient-focused services within a multidisciplinary team.
As a Social Worker II, you will provide social work services to patients and families in Claresholm’s mental health and addiction care setting. The work includes psychosocial assessment, counselling, case management, group therapy facilitation, and discharge planning. You will report to the Clinical Operations Manager while supporting administrative, educational, clinical, and liaison activities.
Key Responsibilities: Facilitate and coordinate client discharge plans
Deliver group work services with the program team
Complete psychosocial assessments and risk screening
Provide individual,



group, and family treatment
Coordinate transitions, resources, and case management services
Requirements: Bachelor’s degree in Social Work
Active or eligible Alberta College registration
At least one year’s recent mental health facility experience
Working knowledge of mental health disorders
Communication, organization, and group facilitation skills
Apply social work expertise, mental health knowledge, and discharge planning skills within Claresholm’s multidisciplinary care environment.
