Support youth stability through intensive case management, care coordination, and advocacy in Peel Region. Work flexibly across Brampton and Mississauga locations.
The Intensive Case Manager empowers youth while assessing comprehensive needs and facilitating timely services. You will foster therapeutic relationships, support care planning, and promote positive development. The work addresses homelessness, mental illness, substance usage, housing, employment, education, and community opportunities.
Key Responsibilities: Promote youth engagement and positive development during care planning
Conduct assessments and use care planning tools
Coordinate wraparound outreach, mental health, harm reduction, and housing services
Build provider relationships and make referrals
Maintain records,



progress reports, statistics, and monthly reports
Requirements: Bachelor’s degree or equivalent education and related experience
Related registration or certification, such as RSW or CYCP
Two to three years of case management experience
Knowledge of Housing First, trauma-informed care, and harm reduction
Ontario driver’s licence, vehicle, insurance, and required certifications
Bring compassionate advocacy, organized documentation, and coordinated support to youth pursuing greater stability through housing and community services.
