Mental Health Housing Workers support mental health, ABI, dual diagnosis, substance use, and housing stability in Peterborough. This full time temporary role requires rotating 24/7 shifts.
At C.M.H.A. H.K.P.R.’s HART Hub, you will provide community-based mental health and housing support. The work is person-centered and multidisciplinary. You will use supportive counseling, case management, and recovery-focused interventions to help clients improve functioning and maintain housing.
Key Responsibilities: Complete assessments and develop treatment, crisis, and safety plans
Provide system navigation, advocacy, referrals, and case coordination
Support relocations, tenancy skills, inspections, and housing stability
Document assessments, treatment plans, case notes, and client records




Collaborate with teams, families, and community partners
Requirements: Three years’ case management or community support experience
Bachelor’s degree preferred, or relevant diploma and experience
Knowledge of mental illness, crisis intervention, ABI, and DD
Person-centered, trauma-informed, and concurrent-informed care abilities
Valid G driver’s licence, vehicle, references, and police check
Support HART Hub clients through crisis intervention, counseling, case management, and practical housing stability work in Peterborough.
