Remote in Canada, support existing Sailor Health patients through advocacy intake using empathy, care coordination, and explicit communication.
As a full-time Advocacy Care Navigator, you will guide patients already working with therapists toward additional advocacy resources. The role combines patient support, emotional guidance, and detailed follow-through. You will help established patients feel supported beyond therapy sessions.
Key Responsibilities: Conduct introductory calls with existing advocacy-service patients
Guide patients through warm, clear advocacy onboarding
Coordinate transitions and follow-through with advocacy teams
Manage follow-ups, missed calls, and patient reassignments
Maintain accurate records in Google Workspace, Healthie, and HubSpot
Requirements: Empathetic communication during vulnerable conversations
Clear listening skills that help patients feel at ease
Strong organization across multiple patient stages
Comfort learning Google Workspace, EHRs, and CRMs
Healthcare, customer service, or administrative experience welcomed
Use empathy, organization, and technology skills to guide Sailor Health patients through coordinated advocacy support remotely.
