Provide community nursing in Wawa, Ontario, using patient assessments and care planning. This part time position includes scheduled visits and RN support.
As a Registered Practical Nurse with ParaMed, you will care for patients of all ages in varied settings. You will support comfort at home while working within your scope of practice. The position includes ethical nursing care, patient engagement, documentation, and care plan monitoring.
Key Responsibilities: Complete scheduled community visits within your nursing scope
Assess individual patient needs using appropriate techniques
Execute and continuously monitor patient care plans
Build rapport and trust through collaborative engagement
Maintain professional records of encounters and care plans
Requirements: Current CNO registration as a Registered Practical Nurse
Valid driver’s license and access to a vehicle
Reliability and flexibility for community nursing visits
Willingness to travel between patient residences
Required on-call coverage, including after hours
Support patients at home through community visits, assessment, care planning, documentation, and cooperative nursing practice.
