Lead community nursing support across Mississauga with Spectrum Health Care, using patient assessment and multidisciplinary collaboration skills. This is full time work.
As a Home Support Supervisor, you will help deliver integrated, personalized support through Spectrum’s Integrated Care Program. The position involves community-based leadership, patient care planning, staff management, and outcome monitoring. You will collaborate with multidisciplinary teams while supporting care delivery in Ontario.
Key Responsibilities: Travel to patients’ homes throughout the community
Assess patient needs and formulate individualized care plans
Manage staff, appraisals, and progressive disciplinary action
Analyze patient and employee feedback with key indicators




Address and document patient concerns and complaints promptly
Requirements: Current College of Nurses of Ontario registration in good standing
At least three years’ progressive leadership experience
Valid driver’s license and reliable vehicle access
Strong interpersonal and difficult-situation management skills
Excellent English communication and community nursing knowledge
Support Spectrum Health Care patients through community nursing leadership, care planning, staff supervision, and effective multidisciplinary communication.
