This Social Worker supports weekday and weekend inpatient acute care programs at Toronto General Hospital. Under the Manager’s direction, you will collaborate with an interdisciplinary team while evaluating service needs and psychosocial functioning. You will also develop recommendations, treatment plans, and discharge plans for patients.
Key Responsibilities: Evaluate patient needs and psychosocial functioning
Formulate recommendations, treatment plans, and discharge plans
Coordinate interventions with patients and family members




Exchange patient information with community agencies
Support effective operation of the Social Work group
Requirements: Master’s degree in Social Work required
Registration with Ontario’s social work college required
Hospital acute care experience required
Solid interpersonal, organizational, and time management skills
Social welfare policies, legislation, and services knowledge
Support patients and care teams through clinical assessment, psychosocial intervention, and coordinated discharge planning at Toronto General Hospital.
