As a Community Care Giver, you will help individuals manage chronic conditions while remaining safely at home. Your work includes personal care, medication reminders, meal preparation, mobility assistance, and emotional support. You will also monitor health changes, maintain records, and communicate with supervisors, clients, and families.
Key Responsibilities: Assist with bathing, dressing, grooming, and personal hygiene
Provide medication reminders and support prescribed treatments
Prepare nutritious meals and assist with feeding
Support mobility, transfers, and assistive device use




Monitor vital signs and report health changes
Requirements: Previous caregiving experience, preferably with chronic illnesses
