CBI Home Health seeks a Registered Nurse for its Integrated Care Program with Southlake Health. You will provide hospital-level nursing care at home for patients with acute and complex needs. The work includes clinical assessment, IV therapy, medication administration, wound care, and point-of-care diagnostics.
Key Responsibilities: Deliver advanced assessments and complex treatments in patients' homes
Monitor acute conditions using remote health technologies
Recognize patient changes and coordinate timely care
Collaborate with hospital and community partners




Support safe recovery and avoidable admission prevention
Requirements: Registered Nurse in valuable standing with the College of Nurses of Ontario
Experience providing acute, complex, or hospital-level nursing care
Experience with IV therapy and vascular access devices
Current valid BCLS training and required screening documentation
Valid driver's license, reliable vehicle, and vehicle insurance
Use acute care expertise, digital health skills, and clinical judgment to support patients across Newmarket-area communities.
