Join our team
Southlake Academic Family Health Team (SAFHT) is seeking a collaborative, patient-centered Pharmacist to support our Interprofessional Primary Care Team (IPCT). Our evolving model combines ongoing primary care with team-based, consultative support for patients referred by community primary care providers, particularly those with complex needs. Together, our physicians, nurse practitioners, nurses, social work care managers, coordinators, navigators and other partners work to improve access, continuity and health outcomes.
Services are provided in person, virtually and, when required, in mobile or community settings. We are committed to equitable, culturally safe, trauma-informed care for diverse, vulnerable and equity-deserving populations.
The opportunity
As the IPCT Pharmacist, you will provide clinical pharmacy expertise across both our Primary Care and Team-Based Care programs. You will work to the full extent of your authorized scope, proactively identify medication-related risks, manage and monitor therapy, and partner with the care team to deliver practical, coordinated care plans.
Key responsibilities
- Provide comprehensive medication assessments, reconciliation, patient counselling and evidence-based recommendations for acute, chronic and complex conditions.
- Identify and address polypharmacy, adverse effects, interactions, adherence challenges and therapeutic gaps; support medication optimization, deprescribing and appropriate monitoring.
- Collaborate with physicians, nurse practitioners, RN and social work care managers, RPN coordinators and navigators; participate in complex-care rounds and shared care planning.
- Assume responsibility for medication-related care-plan actions and risk-based follow-up,
including transitions after hospital or emergency department visits.
- Use EMR registries and clinical data to identify patients who need proactive review, high-risk medication monitoring or chronic disease support.
- Help patients overcome medication access barriers by coordinating with prescribers, community pharmacies and community partners.
- Provide pharmacist-led services aligned with scope, program priorities and capacity, which may include smoking cessation, minor-ailment assessment and prescribing, anticoagulation/INR support, bone health and CBT-I.
- Contribute to clinical pathways, quality improvement, education, documentation standards and service development; support learners and ongoing career development as opportunities arise.
Qualification Requirements
- Degree in Pharmacy and current registration in good standing with the Ontario College of Pharmacists.
- Ability to practise independently within applicable legislation, professional standards and organizational policies; prescribing or other expanded-scope activities undertaken only where authorized.
- Strong clinical judgment, medication-management expertise and excellent communication, documentation and collaborative problem-solving skills.
- Experience in interdisciplinary primary care, chronic disease management, complex care or population-health initiatives is an asset.
- Comfort working with diverse patient populations and across evolving care models, settings and clinical priorities.
Location and schedule
Work is based across SAFHT/IPCT clinic sites, with potential travel within York Region and South Simcoe for patient care, collaboration, training or meetings as required. Scheduling may include days, evenings and weekends, based on operational needs.
