We are committed to reducing systemic barriers to employment and our services that have traditionally impacted members from the following groups, Black, Indigenous People, and other racially visible persons, members of the 2SLGBTQIA+ community, and persons with disabilities. If you are a member of one of these equity‑seeking groups, you are encouraged to self‑identify in your cover letter or your resume. Please also feel free to identify your preferred pronouns when applying. ROLE OVERVIEW The Nurse Practitioner, Community Psychiatry and Mental Health Services provides advanced nursing care to individuals experiencing mental health, concurrent mental health and psychiatric concerns. CMHA Halton is building our interdisciplinary clinical team and is seeking a Nurse Practitioner (NP) who is excited by the opportunity to help expand and shape psychiatric and mental health services within the organization.

This role is ideally suited to a clinician who enjoys building new programs, developing innovative models of care, and contributing to the growth of integrated community‑based psychiatric services. This role has a primary focus on psychiatric care using a client‑centered, recovery‑oriented, and culturally responsive approach. The work of the Nurse Practitioner is supported by an internal interprofessional team that provides wraparound support for our youth and adult clients. Externally, the NP and team coordinates with primary care providers, healthcare providers, and community partners to support individuals with complex and diverse needs. This role will support timely access to care, continuity of treatment and a successful transition to a community service model of care. This position will play a significant role in supporting psychiatric services at CMHA Halton with the NP strengthening the care and support clients receive, including new assessments, treatment, after‑care and physical health reviews.
