Deliver proactive patient-centred care as a Clinical Pharmacist with Mint Health + Drugs Meridian in Stony Plain, Alberta. Use prescribing and injections daily. This full time, permanent Pharmacist role supports an established Pharmacist First workflow. Provide medication reviews, chronic disease management, dispensing, verification, and follow-up care. You will also support travel health services while collaborating with pharmacy technicians, assistants, physicians, primary care networks, and community partners.

Key Responsibilities
- Provide medication reviews, prescribing, injections, and chronic disease management
- Assess patients proactively and develop follow-up care plans
- Support the appointment-based travel clinic and clinical services




- Dispense and verify prescriptions accurately and safely
- Collaborate with pharmacy technicians, assistants, and community partners

Requirements:
- Clinical pharmacist registration with the Alberta College of Pharmacists
- Authorization to administer drugs by injection
- Additional Prescribing Authorization
- At least one year of community pharmacy experience
