You'll assess, plan, and implement personalized rehabilitation programs, working closely with clients, families, and healthcare teams to ensure safe, effective, and meaningful care. Educate clients and caregivers on therapy goals, exercises, and adaptive equipment Collaborate with interdisciplinary teams for discharge planning and care coordination Support clients with complex medical conditions in their homes Safety & Quality: Follow infection control protocols and ensure safe use of equipment Maintain clean and organized therapy spaces Administrative & Program Support: Document care activities and maintain accurate client records Participate in quality improvement and professional development initiatives Bachelor's or Master's Degree in Occupational Therapy (Master's preferred) Registered with the College of Occupational Therapists of Ontario (COTO) Experience with complex medical conditions and mobility support (required) ~ Home and community care experience (preferred) ~ Familiarity with mobile work tools and EMR systems Creative problem-solving and critical thinking abilities Committed to equity, inclusion,

Do you have a university degree in Occupational Therapy from an accredited university?

Do you have a Masters Degree in Occupational Therapy?

Are you eligible for membership in Ontario Society of Occupational Therapists (OSOT) and/or Candian Association of Occupational Therapists (CAOT)?

Do you have a current Basic Cardiac Life Support (BCLS) for Health Care Professionals (HCP) certificate?

How many years of experience do you have working as an Occupational Therapist?

Do you have experience working with clients with multiple complex medical conditions in an acute physical medicine / acute mental health environment particularly with older adults?

Do you have previous experience working in Home or Community Setting?

Do you continually participate in career development and training in your profession as an Occupational Therapist? Are you proficient in writing reports and assessments, measuring workloads and accessing information through excellent computer and MS office skills necessary to be successful in the role?

Do you have experience using mobile work tools, devices and apps required for a community position?

This position requires travel within the HNHB boundaries including Hamilton, Burlington, Brantford, Niagara regions and Norfolk County. Are you willing and able to travel in these locations?
