Support Phyllis Tuckwell patients, families, and carers as a Senior Hospice Nurse focused on clinical leadership and coordinated palliative care.

This leadership role works across inpatient and community services within the Phyllis Tuckwell multidisciplinary team. You will assess changing needs, develop care plans, implement management plans, and evaluate outcomes. The post also supports colleagues through advice, education, supervision, and career development while promoting safe, individualised care.

Key Responsibilities:

- Provide person-centred nursing care for patients and families
- Assess changing needs and refer to appropriate specialists
- Act as nurse in charge on shifts when required
- Support inductions, appraisals, training, and reflective discussions




- Identify risks and report incidents or concerns promptly

Requirements:

- Passion for professional standards, patient safety, and practice development
- Ability to support multidisciplinary relationships and integrated clinical care
- Understanding of NMC guidance and PT policies
- Knowledge of safeguarding, risk management, and health and safety
- Ability to maintain accurate patient records and medicine management standards
