As a Nurse Practitioner, you will evaluate patients, create personalized care plans, and coordinate referrals throughout their virtual care journeys. You will collaborate with doctors, nurses, psychologists, care coordinators, and care managers. The position supports primary care and ongoing patient health management through Dialogue’s Integrated Health Platform.

Key Responsibilities:

- Conduct remote assessments through the Dialogue Care Platform
- Diagnose common and complex conditions and provide treatment




- Identify urgent symptoms and deliver timely medical advice
- Create personalized care plans for patient needs

Requirements:

- At least two years of experience
- Master’s degree in Nursing and PHCNP Diploma
- Valid, active Nurse Practitioner license in Quebec
- Comfort consulting in French or English
- Computer ease and interest in continuous learning
