The Nurse Field Case Manager provides ongoing assessment, planning, evaluation, and advocacy for patients and healthcare providers. This role supports disease education, adherence monitoring, side-effect management, and training across in-clinic, in-home, long-term care, and hospital settings. You will collaborate with pharmaceutical representatives and the reimbursement team.

Key Responsibilities:

- Manage assigned patient and physician or clinic caseloads
- Deliver training to patients, providers, and care partners
- Coordinate communication among physicians, pharmacies, and program teams




- Maintain professional documentation and identify care changes
- Complete required training, studies, and quality improvement activities

Requirements:

- Neurology or Parkinson’s disease experience is an asset
- Five or more years of direct clinical experience
- Current unrestricted Registered Nurse license
- Bilingual French and English communication ability
- Leadership experience and ability to work under pressure
